The Boring Department Running Your Whole Business

So much of marketing is face forward... it's the part everyone sees - the posts, the website, the way you talk about your work out loud. There's a department in your private practice that lives in exactly the opposite direction though. It's not face forward and it's not the thing anyone screenshots or comments on. Most of the time nobody even knows it's there.
Operations is the part of your practice nobody sees. It's your EHR or Electronic Health Record system... the system running quietly in the background while you're in session, completely unaware that a reminder text just went out, a form just got signed, or a card just got charged.
But here's the thing about your operations department that nobody sees... when it's off, clients feel it.

Choosing an EHR for Your Child Therapy Private Practice
Remember, whether you're a practice of one (like me) or a big group practice, your operations department includes the EHR for your child therapy private practice, and that one system can stretch to cover your scheduling, your intake forms, and your payment processing too. It's easy to lump all of that under "the software I pay for" and never think about it again until something breaks. But this business infrastructure can be the difference between a practice that runs while you're not looking at it and a practice that stalls every time you step away.
How I Chose My EHR for My Child Therapy Private Practice
When I opened my business in 2019 I did a lot of research into what I wanted my EHR to do for me. Some practice owners shared their schedule system and processing system were on two different platforms. Others shared they still used paper intake forms locked away in filing cabinets (which I thought was wild in the 21st century!).
I specifically wanted an all encompassing system that could send appointment reminders on its own. As a cash pay practice, that mattered more than people expect. A missed session is a missed payment, full stop. I wanted somewhere I could process cards without an extra step on a separate app and login. I also wanted clients to be able to schedule or reschedule themselves without a hundred emails back and forth.
Even when I was just starting out, I didn't blink at the cost. Not that it was outrageous, I just knew spending the money up front meant I wasn't going to be losing money somewhere else... in no-shows, in hours spent chasing payments, and in forms that never made it back to me before the first session.
What SimplePractice Actually Handles for Me
This is the system I landed on and it's still what I use.
Forms. I love the templates SimplePractice already has built in, and how easy they are to customize and make my own. Intake paperwork that can take others an afternoon to build from scratch just takes a few edits on SimplePractice.
Reminders. Appointment reminders go out without me lifting a finger, and they've done more for my income than I expected. When a client has to cancel last minute, more often than not they'll say something like, "I got the reminder, I just couldn't make it - it's okay, you can charge me." That's a client who already knew the policy, already had the heads up, and didn't feel blindsided by a bill. The system doing that quiet work in the background is worth more than people give it credit for.
Payments. Cards get processed in the same place everything else lives. I can charge manually after a session, or set everything to run automatically, whenever works best for me. No separate system or password to track down.
Scheduling. I schedule clients at the end of their sessions, but they can also book or reschedule themselves whenever they need to, without a single email back and forth.
It makes me look like I have a team behind me, when it's really just me behind every piece of it - which is exactly the point.

What I Don't Use and Why That's Okay Too
I don't submit to insurance through my EHR because I run a cash pay practice, so it's a feature I don't pay for or think about. I also don't use AI during sessions. A lot of therapists love that option - and I understand why - it's just not something I'm for. A system being able to do everything doesn't mean you have to use everything it offers. Pick what actually serves your practice and leave the rest on the shelf.
Two More Systems Worth a Summer Look
While you're in your operations department tightening things up, here are two more corners that deserve a few minutes each. First, your password and backup setup, when's the last time you actually changed a password or confirmed your documents are backed up somewhere outside your laptop?
Second, optimize whatever you use to track your own admin work - the tasks only you know need to happen. I've cycled through different systems over the years. I ran on Monday.com for a while and loved it. I've heard good things about Asana and Trello too. Right now I keep it simple and use Google Tasks and my reMarkable notebook, which I genuinely love!
It's fine to outgrow a system, or just get tired of it and land somewhere simpler. Summer is a good time to make sure whatever you're using right now is somewhere you'll actually look at it, not a sticky note that fell behind your desk in March.
